Primary Health Services Division is the convener of the Sacramento County Medi-Cal Managed Care Advisory Committee and the Care Coordination Work Group. Staff assist in the development and maintenance of the resources shown. All resources are validated prior to posting. However, as changes can occur, please contact the plan or provider to confirm information.

Most Medi-Cal beneficiaries are required to enroll in a Managed Care Health Plan. Sacramento County uses the Geographic Managed Care (GMC) model, in which the State Department of Health Care Services (DHCS) contracts with several commercial health plans. The Health Plans currently available in Sacramento County are:
- Aetna Better Health
- Anthem Blue Cross
- Health Net Community Solutions
- Kaiser Health Plan
- Molina Healthcare
- UnitedHealthcare Community Plan - Important: This plan will exit Sacramento GMC on 10/31/18. The State and UnitedHealthcare will begin outreach to enrollees by August 1 to inform them of their choices and to assist in making the transition.
New Medi-Cal recipients must choose a Health Plan or will be assigned to a plan through a default process. Plan enrollment is processed by
